De novo Crohn's disease and infectious proctitis due to cytomegalovirus were recently discussed.


Two case reports involving gastroenterology were published Dec. 19, 2023, by Annals of Internal Medicine: Clinical Cases.

The first report described a 55-year-old man who was found to have de novo Crohn's disease nine years after laparoscopic sleeve gastrectomy for medically complicated obesity. Most previous cases of this disorder have been reported in women who have had Roux-en-Y gastric bypass, the authors noted.

In the second report, an 83-year-old woman with no other traditional immunocompromising risk factors presented with cytomegalovirus (CMV) proctitis. The authors advised clinicians to maintain a high index of suspicion for CMV in the presence of typical endoscopic findings, even in patients who are not immunocompromised.
