Recent cases include massive empyema with Edwardsiella tarda in a patient with cirrhosis, paraneoplastic myositis in a patient with extrahepatic cholangiocarcinoma, and two patients with refractory iron deficiency anemia secondary to angiodysplasia-related GI bleeding.
Two case reports and a case series on gastroenterology and hepatology topics were published April 4 by Annals of Internal Medicine: Clinical Cases.
The first report described a 78-year-old man with liver cirrhosis who regularly ate raw fish, was emergently admitted for treatment of septic shock, had massive empyema in his right lung shown on CT, and was diagnosed with septic shock with empyema caused by Edwardsiella tarda. The second case report described paraneoplastic myositis resulting in acute renal failure in 71-year-old man who presented with a pancreatic mass that was ultimately diagnosed as an extrahepatic cholangiocarcinoma. Finally, the case series described a dramatic, fast, and sustained response to bevacizumab in two patients with GI angiodysplasia and rapidly recurrent iron deficiency anemia secondary to chronic GI bleeding who continued to be symptomatic despite multiple pharmacologic and supportive interventions.
